Documentation for students who were unable to test online
The form linked below
should be used to document any and all reasons for NOT
testing students online. The documentation is for your
files and should NOT be submitted to DWE.
The Technical Assistance
Team will want to view this documentation when they visit
your school, make sure you have it available for them.
The form
should be used to document the following:

Technical issues that
kept your students from testing. All technical issues
which kept your students from testing should be noted as
either a “school issue” or as a “Questionmark issue”.
The only time that a Questionmark issue should be noted
is when you have an official CTE SCT Listserv post that
you can print and attach to the documentation form.

Special Needs students
that will not be tested. The only time this should be
noted is if the student meets the criteria as stated in
the Policy for Testing Special Needs Students.
- Other
anomalies that occurred during testing.

by DWE.
